About 245D Services

245D services are licensed home and community-based supports in Minnesota designed to help individuals with disabilities or special needs live as independently and safely as possible. These services provide personalized care, supervision, skill development, and community integration — all while protecting dignity and promoting quality of life.

Basic Support Services

  • Respite Care – Short-term care to provide relief or coverage during the absence of a family member or primary caregiver.
  • Night Supervision – Overnight staff support and monitoring in the individual’s home.
  • Individualized Home Supports (Without Training) – Assistance, supervision, and support in the person’s own home (or family home) — including household management, health & safety monitoring, adaptive skills, and promoting community participation.
  • Homemaker Services – Help with light household tasks, home management, and daily living activities (e.g. cleaning, laundry, errands).
  • 24-Hour Emergency Assistance – On-call counseling, problem-solving, and immediate response for health or personal crises in the home.

Intensive Support Services

  • Individualized Home Supports (With Training) – Habilitation training and structured support in domains such as health, safety, daily living skills, and community engagement — delivered in the individual’s home.
  • Individualized Home Supports (With Family Training) – A holistic approach that includes support and training for both the individual and their family to promote consistency, skills development, and successful community living.
